| Brian remains unconscious. He was operated on mid-day for vascular circulation in his right leg, which was corrected. He is not expected to regain consciousness for another day. The doctor's concerns at this point are liver and kidney function, as well as tissue damage in his right leg. It is expected to be 3-5 days before the doctors are able assess the full extent of the damages. |

| Brian is recovering from vascular surgery on his right leg that was performed earlier this afternoon. He now has a faint pulse in his right foot. He has begun to regain consciousness, but has been sedated for his protection. We expect the respirator to be removed on Saturday and his vitals are now stable (90/50). |

| Brian regained consciousness, but has been sedated because his breathing is still being supported by a respirator and due to the pain his right leg. His blood pressure is 107/52. His pulse is 119. He has regained a pulse in his right foot, but he still has significant swelling in his legs. Because of the breakdown in the leg muscle tissue, he continues to give off toxins that are going to his kidneys. He is currently being treated with blood transfusions and high doses of fluids. He continues to have high levels of blood and fluid loss from his legs. His liver levels are slowly declining, which is a positive sign. His kidney levels are still high and of concern. Hopefully, he will be taken off of the respirator early next week and we hope to know the extent of the kidney, liver, and leg tissue damage by the later part of next week. Brian is currently resting and being visited by his immediate family. |

| Brian is awake. He is still on the respirator, because he cannot take in enough oxygen on his own. There is concern of pneumonia from being on the respirator for so long. He is being given medication to minimize any pain he may be feeling. There is also a possibility that he may have to undergo dialysis, but the doctors will be reducing the amount of fluids they are giving him before they take any steps in that direction. |

| Brian is awake but very tired. His vital signs are better but he has a fever of 100 degrees. He is being given a pain killer for the legs. The lab numbers for his kidney and liver are high and are not trending down yet. He has been given medication to improve the urine out put which is a concern. The oxygen level on his respiratory has been lowered to in hopes to wean him off the machine. |

| Brian was awake yesterday laughing and joking. His vitals have improved. Hopefully, he'll be taken off the respirator Today or Tuesday. The doctors are still concerned about the damage to his kidneys, liver and legs, but will not know the extent of the damage until later in the week. |

| Brian is experiencing significant pain in his legs. The doctors are trying to reduce the amount of pain medicine and sedatives in order to wean him off the respirator. His blood pressure and heart rates are elevated and are a concern as well. The ICU staff has asked visits to be limited to the immediate family for only short periods so he can get the rest he requires. |

Brian’s condition improved overnight. He is awake and semi alert. He is putting out more urine indicating an improving kidney function. The lab works on his Kidney and liver are still high but trending downward. He has a good distal pulse in his left leg and able to move it. His right distal pulse is getting stronger but he has not moved the leg. The hospital hopes to remove the breathing tub from his throat and take him off the respirator today.

Unfortunately the Doctors were not able to wean Brian off the respirator today. The dressings on his legs were changed today which required administering a sedative for the leg pain. This caused his respiration to slow down preventing the removal of the airway. His Kidney and liver numbers are trending down which is a good sign. His blood pressure is high at 170/80 and pulse is better at 112. The ICU will attempt to remove the respirator on Wednesday.

Brian’s condition continues to improve. His kidney and liver numbers are high but trending downward. He has no fever, but is being given an antibiotic as a precaution. His pulse and blood pressure are high, but he is awake and feeling anxious from the breathing tube. The doctors will attempt again today to remove the respirator and breathing tube. The vascular specialist examined Brian’s legs yesterday. Although he said Brian’s case was the most serious he was currently handling his progress is very good. His circulation continues to improve in both legs. He will require plastic surgery on the left leg sometime down the road.

| Brian was taken off the respirator and is now breathing on his own. His is giving off urine without the help of medication. Unfortunately, Brian’s liver and kidney numbers are still very high but slowly trending down. He is being sedated for the pain from both legs. |

Brian was moved off the ICU late Thursday afternoon. He is in extreme pain from the incisions in both legs and has pain coming from his lower stomach. The doctors have upped the pain management to morphine and will be doing a CAT scan of his stomach today. He is awake but semi alert.

| Brian had a difficult day with the pain in his legs and stomach. A CAT scan of his stomach was negative with no signs of blockage. His pain medication was upped to morphine every 4 hours. The vascular surgeons’ have scheduled surgery on his legs Monday to remove scare tissue. He is uncomfortable but his blood pressure and pulse are getting closer to normal levels. Brian’s liver and kidney lab numbers continue to trend down. |

| Brian’s condition continues to improve. His vitals are higher than normal but in an acceptable range. His liver and kidney numbers are declining but still too high. He had physical therapy on his legs today for the first time. It was very painful but the beginning of a long recovery for the legs. He is in a great deal of pain from the legs which results in him not being able to sleep and acting restless. Brian is being given morphine every four hours for pain control. |

| Brian was visited by the vascular surgeon and neurologist this afternoon. They tested the reflexes in his right and left legs. He still does not have any feeling in his right leg below the knee. The doctors will continue to test his legs, but believe that it may be a few weeks before he regains feeling in the lower half of his right leg. They will have a better idea following the surgery that will be performed on his right left on Monday. |

Brian experience severe chest pains today. The doctors ruled out a cardio problem and are treating the problem as a blot clout in the lungs. He is now in an isolated room due to his infection. So far, the surgery for his legs is scheduled for Monday.

Brian had surgery on both legs late this afternoon. The vascular surgeons found more dead tissue in his right leg than expected. The continued break down in his leg tissue resulted in more toxins flowing to his weak kidney. Brian will require additional leg surgery to remove bad tissue the timing for it is unknown. The left leg is in better shape and the doctors disconnect the suction machine from the leg. The hospital will be giving him a unit of blood tonight. The prognosis for the legs will depend on how the tissue and nerves heal. He is continuing to be treated with a blood thinner for the lung clot. This treatment increases his overall body fluids delaying the heeling in the legs. The lab studies for the liver and kidney are continuing to trend down but we are still not sure how much damage has been done to the organs. His heart rate, blood pressure and oxygen levels are improving. He is being given morphine for the leg pain. He is having difficulty sleeping and is very tired. Brian is feeling anxious and is experiencing altered mental status.

| Brian continues to be in considerable pain and is being medicated with morphine. This has caused him to have an altered mental status. He was able to sleep today. The doctors are discussing putting a screen in his right leg to prevent blood clots in his lungs. This is an alternative to treating him with fluids which have overstressed his kidneys in prior treatments. The attending physician is consulting with an infectious desease specialist to determine the best medication for treating Brian's intestinal infection. The vascular surgeon will consult with a plastic surgeon for treating Brian's legs. Brian was giving a unit of blood last night to compensate the blood loss in his legs from last night's surgery. The attending physician does not feel that Brian will beed dialysis and that any issue issues with his kidneys will be treated by adjusting his diet. The liver lab studies are improving and the attending physician feels that the liver will be ok. |

| Brian's kidneys have begun to reach normal levels. His intestinal infection is being treated with medications. Brian's legs are still a major concern, particularly the right one. The vascular doctors have decided not to put a screen in Brian's leg to prevent further clots. Instead they will continue to treat the clot in his lung with blood thinners. In the coming days, the vascular doctors will weigh their options as to the steps to take with Brian's right leg. There remains to be no feeling below the knee. Brian's mental state was better today as he was laughing and joking around which is good to see. |

| There have been no significant change in Brian's medical condition since Thursday. However, the doctors are very concerned about the ability to save his right leg below the knee. Various options are being considered. The doctors plan to do surgery on Monday for removal of additional dead tissue in the leg. |

| Brian's condition continues to improve. His leg is a major concern still. Discussions continue of whether or not amputation of his right leg is the best option. We have contacted surgeons at the Hospital for Special Surgery and Yale to render opinions. Time is critical because we are concerned of infection in the leg. |

Brian’s condition remains unchanged. He is pale and feeling cold which is believed due to the fact he is breaking a fever. The doctors have delayed Monday’s surgery to remove dead tissue from his leg. Instead they have scheduled a nerve connectivity test to ascertain the viability of Brian’s leg nerves.

| The Vascular Surgeon saw a marked improvement in Brian's right leg tissue. As of today the Doctor has temporarily pulled amputation off the table. An MRI was ordered to determine the extent of damage to the tissue and nerves. If the MRI is not clear the surgeons will go ahead with a nerve conductivity test. |

The vascular surgeon preformed a second operation on Brian’s right leg tonight. The surgeon removed “a lot of dead tissue from the leg”. While the wound was open the Orthopedic and Neurological Surgeons evaluated the site. The Doctors still are ruling out amputation of the right leg at the knee for now but are concerned about its functionality. They did not find any gross infection in the leg which is a good sign.
Brian is complaining of pain coming from his right foot. If this is real pain then it is a sign that the nerves are rebuilding. The Doctors plan to do a nerve connectivity test and MRI this week on his right leg. The left leg continues to heal and will need require plastic surgery.
The liver and kidney continue to improve.

Following Monday’s operation Brian developed a fever which required him to stay in the post op area for most of the night. Tuesday was a difficult day as he spiked a fever of 101 degrees and began vomiting. The Doctors are trying to determine the cause of the fever. He had to have his new IV port which was changed on Tuesday replaced. As that was not enough Brian developed a blockage in the suction to his right thigh which required the vascular surgeon to replace the lines.
Brian has not taken the MRI or nerve connectivity test yet.

| There have been no significant change in Brian's condition. He is being treated for a yeast infection. The orthopaedic surgeon is cautiously optimistic about saving Brian's leg and its functionality following the last surgery. A third operation is planned for next week to remove more dead tissue. He is still on heavy pain medication which makes him "loopy." |

Brian is alert but on heavy pain management. He is having a problem handling the pain medicine as it is causing him to vomit. The Vascular surgeon changed his leg dressing Friday evening and said they will consider further surgery to remove dead tissue in the right leg. Following the last surgery the orthopedic surgeon said he was opportunistically cautious that the leg could be saved and he would have some function in it. Saturday they plan to do an MRI to see how the nerves are repairing.

Brian’s condition is steadily improving. He continues to spike fevers but the doctors believe it is a fungal infection from the antibiotics. He had an MRI this evening to ascertain the viability of the nerves in his right leg. The results should be available in the next 24 hours. The viability of his right leg below the knee is still questionable and a third debridement is tentatively planned for this week.

| Tonight, due to a significant infection, the doctors amputated Brian's right leg below the knee. They plan to do a second surgery to remove a portion of the leg above the knee at a later date. This a preventative measure against septic shock. He will be in post-op until some time tomorrow. |

Brian was moved from the recovery unit to ICU late Friday afternoon. He is resting comfortable through heavy pain management. He is alert and aware of what is going on around him. His spirit is considerable good under the situation. The doctors are monitoring him for the possible spread of infection from the amputated leg and potential for pneumonia. A second operation to remove more of the leg is planned in the future.

Brian developed an infection today which has prevented him from being moved from the ICU. The doctor’s are treating the infection with a new antibiotic. If the new drug works he will be moved back to the medical floor on Monday. He is very tired from the pain medication and is sleeping most of the time. The doctor’s are adjusting the pain meds to keep him more alert as they are concerned about fluid filling up in his lungs. He has been experiencing phantom pain from the amputated right leg which was expected.

Brian remains in the ICU due to his infection. He was alert today and did not feel as much pain. The Vascular Surgery Team changed the dressing on his right leg and was happy with the healing progress. He was able to move his right knee when asked to do so by the doctors. Unfortunately, Brian has been vomiting quite a lot it is thought due to the pain medication. The Doctors are experimenting with different meds and amounts of the pain killers in hopes of alleviating the nausea. His temperature and vitals are trending near normal.

Brian was moved from the ICU to the medical floor Tuesday afternoon. He continues to fight an infection which is being treated with antibiotics. His pain is under control with the latest pain management regiment. Brian’s vitals are treating closer to normal levels. He is alert however sleepy from the drugs. His attitude is subdued and is beginning to be seen by a therapist.

Brian is resting comfortable. His pain is under control with the medication. The infections continue and the doctors have added another antibiotic to the mix. The Vascular team is satisfied with the drainage from his right leg. They do plan to perform another surgery next week to clean out more dead muscle tissue.

Brian had a good day today. The physical therapist got him out of bed for the first time today. They worked on him standing on his leg using a walker for support. The therapist also worked on having him strengthen his right leg. Brian sat in a chair for the first time as well. All of this is a step forward for Brian.
The vascular surgeon saw Brian today and said that he was happy the way that his leg has been draining. Brian's left leg has already been closed up. The doctor will meet with the plastic surgery team and decide when and how they will close Brian's thigh, as well as the stump. As of now, his leg is healthy and the doctors are positive about its recovery without having to further amputate. |

| Sunday was a good day for Brian. He was very alert and not in too much pain. He continues to experience severe vomiting despite nausea medication. The vascular team examined Brian Sunday afternoon and indicated they would do another debridement of the right leg Wednesday or Thursday this week. If all goes well the plastic surgeons will close the stump below his right knee and right thigh. |

| The surgeons closed Brian's stump during surgery today. They grafted skin onto his right thigh. He is in a lot of pain, but it's being controlled with medication. The doctors feel good about Brian's recovery. |

Brian’s condition is steadily improving. The doctor’s are reducing his pain medication and he is more alert. Both the stomp and right thigh have been closed up by the surgeons and there does not appear to be any infection in the leg. The Doctor’s are very satisfied with how well the leg is healing. Barring any unforeseen event Brian will be moving to a rehabilitation facility in the next two weeks.
His mood is understandably somber and he is being receiving professional help so to emotionally heal.

Brian is slowing recovering. He is unable to get out of bed as the plastic surgeons want the donor skin sites on his left thigh to heal. His pain is under control with the pain management. He is very tired and taking cat naps. Understandably, Brian is feeling down about his leg and the long stay in the hospital. If he continues to improve the doctors hope to release Brian to a rehab center sometime next week. |

Brian continues to improve. The hospital physical therapist is getting him out of bed for a few hours each day. He is only connected to his IV lines for short periods and now takes pain medication orally. Brian is no longer in isolation as the infection appears to be addressed. Hopefully Brian will be able to move to a physical rehab facility his medical condition and availability of a bed.

Casey and her college roommate Kate were able for the first time to take Brian outside his hospital room. They trolled around the main and the children’s hospital. Brian tried playing the piano in the hospital lobby but unfortunately he realized he did not have a right leg to work the piano pedal. Brian’s electrolyte level is now in the normal range barring unforeseen complication he should be able to go into rehab. He is awaiting final approval from Helen Hayes Hospital in Nyack for his physical rehab.

Brian was released from the hospital this morning and has been moved to Westchester Medical Center's "Tyler Rehab Center" located accross the street from the hospital. Brian has been in physical rehab for two weeks and has been working hard at rebuilding his strength. He was fitted with a temporary prosthesis last week and has been learning to walk again. We expect Brian to be released from the rehab center this Friday March 26th. He will continue doing therapy as an outpatient for several more weeks.
